Indonesian population has high ethnic diversity with a population of about 225 million with NPC incidency rate of 6.2 per 100,000 population per year. EBV infection is not found in all normal cell of nasopharyngeal epithelium, however it can be found in all NPC cells. Protein BRLF1-EBV (Rta) and protein BZLF1-EBV (Zta) act a main transcription activator in EBV infection, particularly in immediate-early lytic phase of EBV lytic cycle. Expression of mRNA BRLF1 EBV specifically associated with NPC tissue tumor patient. This study aims to investigate a potential use for relative expression of mRNA BRLF1 EBV as a biomarker of NPC tumour progressivity. The research was done from Januari 2009 till Januari 2012. This reseach design was cross sectional study. The subjects of research are patient who have been diagnosed of having NPC. The sample used in this research is NPC biopsy tumour tissue from Departement ENT, Dr. Cipto Mangunkusumo Hospital - Faculty of Medicine Universitas Indonesia. All research subject were given informed consent before taking part in this research. Total of research subject were 67 people which we consist of 2 people for NPC WHO-1, 4 people for NPC WHO-2, 46 people NPC WHO-3 patients and 15 healthy people for control. The expression of mRNA LMP1 and that of mRNA BRLF1 in patient NPC WHO-3 was determined by RT-PCR technique. The relative expression of mRNA BRLF1 in patient NPC WHO-3 expressed mRNA LMP1 EBV was determined by RT-qPCR technique and calculated using 2-ΔΔCt. The positivity of expression of mRNA LMP1 was 42/46 samples (93,1 percent) and that of expression of mRNA BRLF1 was 30/46 samples (65,2 percent). The median relative expression of mRNA BRLF1 on status M1 (after distant metastases) has signficantly increased (p = 0,005**) than on status M0 (before distant metastasis). The median relative expression of mRNA BRLF1 in advanced stadium in NPC distant metastases (stadium IV-C) has signficantly increased (p = 0,007**) than in advanced NPC before distant metastases (stadium III, IV-A, IV-B).
